Medical Restrictions Definition:
A medical restriction applies to a physical or mental impairment that may restrict an employee from fulfilling one or more specific job responsibilities.
Medical Restrictions are obtained from one of two third party administrators Centurylink utilizes; Sedgwick for Workers' Compensation claims and Reed Group (Centurylink Disability Services - "CDS") for "off job" medical restrictions claims. (ALL EMPLOYEES)
- Medical restriction requests for worker's compensation related injuries should be reported to UNICALL at 866.864.2255.
- Requests for medical restrictions not related to an on job injury should be reported to CDS at 800.729.7526. See guidelines for requesting medical restrictions with CDS below.
